Terms of Service
These Terms of Service ("Terms") govern your use of therapy services and this website provided by Axis Mental Health. By scheduling a consultation or appointment, you acknowledge that you have read, understood, and agree to be bound by these Terms.
1. Therapy Services
Professional Relationship
Axis Mental Health provides outpatient mental health counseling services. Mitchell "Mitch" Olson, MA, LPCC is a Licensed Professional Clinical Counselor (LPCC) licensed in Minnesota.
Scope of Services
Services include individual therapy, couples therapy, and related mental health counseling. Services are provided via telehealth and in-person in St. Louis Park, Minnesota.
Not for Emergencies
Therapy services are not appropriate for mental health emergencies. If you are experiencing a crisis, please call 988 (Suicide & Crisis Lifeline), go to your nearest emergency room, or call 911.
Minnesota Residents Only
Services are only available to residents of Minnesota. By engaging in services, you confirm that you are physically located in Minnesota during all telehealth sessions.
2. Appointments and Scheduling
Scheduling
Appointments can be scheduled by phone ((612) 562-9880), email (mitch@axismentalhealth.com), or through our contact form. We will confirm all appointments.
Cancellation Policy
If you need to cancel or reschedule an appointment, please provide at least 24 hours' notice. Cancellations with less than 24 hours' notice or missed appointments (no-shows) may be charged the full session fee unless there is an emergency.
Late Arrivals
If you arrive late to a session, the session will end at the scheduled time and you will be charged the full session fee. Please arrive on time to maximize the benefit of your session.
Therapist Cancellations
In the event that I need to cancel or reschedule an appointment, I will provide as much advance notice as possible and will not charge you for the cancelled session.
3. Fees and Payment
Session Fees
The fee for individual and couples therapy sessions is $150 per 55 minutes session. Fees are subject to change with 30 days' notice.
Payment
Payment is due at the time of service. We accept cash, check, credit/debit cards, HSA, and FSA. For insurance clients, copays or deductibles are due at the time of service.
Insurance
If you are using insurance, you are responsible for understanding your benefits, including copays, deductibles, and session limits. We will verify benefits as a courtesy, but you are ultimately responsible for all charges not covered by insurance.
Superbills
For private pay clients with out-of-network benefits, we can provide superbills for you to submit to your insurance company for potential reimbursement. Reimbursement is not guaranteed and is the responsibility of the client.
Late Payment
Unpaid balances over 30 days may result in suspension of services and may be sent to collections.
4. Confidentiality and Privacy
Your confidentiality is protected by law and professional ethics. Information shared in therapy is confidential with limited exceptions as required by law:
- If there is an imminent risk of harm to yourself or others
- Suspected child, elder, or vulnerable adult abuse or neglect
- Court order or legal mandate
- With your written authorization

5. Telehealth Services
Telehealth sessions are conducted via secure, HIPAA-compliant video platform ({axis_telehealth_platform}). By participating in telehealth, you acknowledge:
- You are physically located in Minnesota during the session
- You are in a private, confidential space free from distractions
- You have a reliable internet connection and functioning device
- Technical issues may occasionally occur and may require rescheduling
- Telehealth may not be appropriate for all situations
6. Client Responsibilities
As a client, you agree to:
- Arrive to sessions on time and prepared
- Provide accurate and complete information
- Notify us of any changes to your contact information, insurance, or mental health status
- Communicate openly and honestly during sessions
- Follow the treatment plan and recommendations to the best of your ability
- Pay all fees and charges in a timely manner
- Treat the therapist and staff with respect
7. Termination of Services
Client-Initiated Termination
You may discontinue therapy at any time. We encourage you to discuss termination with your therapist to ensure a smooth transition.
Therapist-Initiated Termination
I may terminate the therapeutic relationship if:
- You repeatedly miss appointments or fail to pay
- You are not benefiting from services or require a higher level of care
- There is a conflict of interest or ethical concern
- You engage in threatening or inappropriate behavior
In such cases, I will provide appropriate referrals when possible.
8. Limitations of Therapy
Therapy is a collaborative process that requires active participation. While many people benefit from therapy, results are not guaranteed. Factors influencing outcomes include:
- Your commitment and engagement
- The complexity of your concerns
- External life circumstances
- The therapeutic relationship and fit
Therapy can sometimes bring up uncomfortable emotions or memories. This is often part of the healing process, but it's important to communicate any concerns with your therapist.

This website is for informational purposes only. Content on the website does not constitute medical advice or create a therapist-client relationship. To engage in therapy services, you must schedule a consultation and complete intake paperwork.
Intellectual Property
All content on this website, including text, images, logos, and materials, is the property of Axis Mental Health and protected by copyright law. You may not reproduce, distribute, or use content without written permission.
10. Liability and Disclaimer
To the fullest extent permitted by law, Axis Mental Health and Mitchell "Mitch" Olson, MA, LPCC shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to therapy services or use of this website.
Therapy services are provided "as is" without warranties of any kind, either express or implied. We do not guarantee specific outcomes or results.
11. Changes to These Terms
We may update these Terms periodically. Changes will be posted on this page with an updated effective date. Your continued use of services after changes indicates acceptance of the updated Terms.
12. Governing Law
These Terms are governed by the laws of the State of Minnesota. Any disputes arising from these Terms or services will be resolved in the courts of Hennepin County, Minnesota.
